Job Description
American Traveler is seeking a Physical Therapist for a Home Health assignment requiring at least 1 year of recent experience, OASIS and HCHB proficiency, and active licensure.
Responsibilities
Home health setting providing care to geriatric patientsCoverage area extends up to a 60-mile radius from the officeAverage of 5-7 patient visits per day with a minimum productivity of 30 points weeklyRequired to complete 6 visits dailyShift is Monday through Friday, days (8 : 00 AM - 5 : 30 PM)On-call rotation required every third to fourth weekendUses HCHB (Homecare Homebase) for documentation; experience is requirediPad provided for documentationMileage reimbursed at the current IRS rateResponsibilities include evaluating, planning, and delivering physical therapy interventions to geriatric patients in their homesFlexibility in covering a wide territory is requiredStandard dress code is scrubs of any color, jeans are not permittedAll documentation must be completed in a timely mannerFirst-time travelers are welcome to applyLocal candidates are accepted at the travel rateAll requested time off must be submitted at the time of applicationMust complete a Hospice / Home Health Checklist for considerationApplicants who were former perm employees must have a six-month separation and receive HR approval before being eligible for this positionTravel pairs are accepted, but each will have their own assigned territory and must have separate transportationRequirements
