Job Description

American Traveler is seeking a Physical Therapist for a Home Health assignment requiring at least 1 year of recent experience, OASIS and HCHB proficiency, and active licensure.

Responsibilities

  • Home health setting providing care to geriatric patients
  • Coverage area extends up to a 60-mile radius from the office
  • Average of 5-7 patient visits per day with a minimum productivity of 30 points weekly
  • Required to complete 6 visits daily
  • Shift is Monday through Friday, days (8 : 00 AM - 5 : 30 PM)
  • On-call rotation required every third to fourth weekend
  • Uses HCHB (Homecare Homebase) for documentation; experience is required
  • iPad provided for documentation
  • Mileage reimbursed at the current IRS rate
  • Responsibilities include evaluating, planning, and delivering physical therapy interventions to geriatric patients in their homes
  • Flexibility in covering a wide territory is required
  • Standard dress code is scrubs of any color, jeans are not permitted
  • All documentation must be completed in a timely manner
  • First-time travelers are welcome to apply
  • Local candidates are accepted at the travel rate
  • All requested time off must be submitted at the time of application
  • Must complete a Hospice / Home Health Checklist for consideration
  • Applicants who were former perm employees must have a six-month separation and receive HR approval before being eligible for this position
  • Travel pairs are accepted, but each will have their own assigned territory and must have separate transportation

    • Requirements

  • Current PT license required if applicable
  • Minimum 1 year of recent home health experience required
  • Active BLS certification required
  • OASIS experience required for Start of Care
  • Experience with HCHB (Homecare Homebase) system required
  • Active driver's license and personal transportation required
  • Ability to work independently and travel throughout assigned territory
  • Timely documentation and communication skills are required
