[job_card.job_description]Master's degree in nursing and graduate of an accredited Nurse Practitioner program Unrestricted New York state license as an ANP or AGNP Unrestricted DEA Certification Experience working with geriatric population and / or long-term care. Eligible to provide reimbursable services and maintain provided status with Medicare and Medicaid Minimum 3 years of experience ; preferably in long-term care Experience with eClinicalWorks (eCW) electronic health records All standard benefits No nights or weekends Opportunity for professional growth and development
Job Title : Adult / Gerontology Nurse Practitioner

Shift : Full & Part-time available
Responsibilities :
- Conduct comprehensive health assessments of residents, including physical examinations, medical histories, and medication reconciliation.
- Develop and implement individualized care plans in collaboration with the interdisciplinary team, considering the unique needs and preferences of each resident
- Order & interpret diagnostic tests, such as laboratory tests and imaging studies, to aid in diagnosis and treatment planning
- Participate in care initiation for our chronic care management services.
