Nursing degree, active registered nurse (RN) license and minimum 1+ years RN experience required. Applicants who do not meet these qualifications will not be considered.

Deliver skilled nursing care to patients in their homes by following physicians orders and agency protocols, focusing on holistic patient assessment, care planning, and coordination. This contract position requires supervision of Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses, ensuring high standards of patient care consistent with state-specific regulations.

Key Responsibilities :

• Conduct thorough patient evaluations and ongoing assessments to determine medical status and care needs.
• Develop, implement, and regularly update individualized plans of care based on clinical findings.
• Supervise, instruct, and evaluate Home Health Aides and LPNs / LVNs, ensuring compliance with care plans and regulatory requirements.
• Maintain detailed, accurate patient records including communication logs and clinical notes.
• Communicate promptly changes in patient condition to physicians and coordinate appropriate interventions.
• Adhere to state-specific nursing standards and home care regulations.
• Attend periodic meetings and home visits; maintain reliable transportation and current drivers license.
