Hospice of the East Bay (HEB) is a community-based nonprofit that provides the hospice and palliative care services that support and comfort people and their loved ones through the final stages of life.

The coverage areas for patient care are : Livermore, Orinda, Pleasant Hill, Walnut Creek, San Ramon, Alamo, Danville, Pleasanton, Dublin, Moraga and surrounding cities.

POSITION SUMMARY :

Under the direction of the attending physician and Medical Director (or Hospice Physician, Nurse Practitioner) and in conjunction with the interdisciplinary group, the main role of a Registered Nurse (RN) is providing direct nursing care and coordinating related assistance for patients and families.

ESSENTIAL FUNCTIONS :

• Revisit Nurse : assist with needed revisits
• Case Management : assist with covering absences for a case manager caseload and attend IDT as needed.
