Position Overview This role involves coordinating services to optimize patient outcomes through assessment, evaluation, collaboration, and management of care delivery systems. The focus is on implementing cost-effective strategies that ensure high-quality care within the current healthcare environment for the designated patient population.
Key Responsibilities - Maintain a safe and clean work environment, adhering to unit safety and infection control protocols.
- Coordinate patient services to maximize outcomes effectively.
- Assess, evaluate, and collaborate with healthcare teams and participate in service line activities.
- Manage utilization, discharge planning, and other healthcare service systems.
- Serve as a liaison and facilitator among patients, families, healthcare providers, and insurance personnel.
- Oversee direct admission processes and bed assignments for patients admitted through physician offices or transfers from other facilities (emergency department focus).
- Provide coverage for holiday shifts as required.
Qualifications - Minimum of three years clinical and case management experience in an acute care setting preferred.
- Familiarity with Prospective Payment System, JCAHO standards, and regulatory requirements related to Utilization Management and Discharge Planning is desirable.
- Strong written and verbal communication skills.
Education - Bachelor's degree in nursing or a related health field preferred.
Licenses - Current Registered Nurse license required.
Successful candidates must pass a physical exam, drug screening, and background checks prior to employment. This position is located in a regional healthcare setting within the Pacific Southwest region of the United States.
