Job Description

• Craft Tailored Therapy Plans : Assess patients' needs and create personalized strategies designed to help them reach their functional aspirations
• Implement Therapeutic Measures : Apply evidence-based approaches to enhance patients' skills, cognition, and ability in daily activities
• Collaborate with Multidisciplinary Teams : Work hand-in-hand with OTs, PTs, STs, physicians, and nurses to ensure holistic, patient-centered care
• Guide Patients and Caregivers : Provide education on therapy techniques to help patients and families support continuous progress at home
• Track and Assess Progress : Monitor clinical outcomes and adapt treatment plans as necessary, ensuring patients remain on their journey to recovery
