Travel Nurse Opportunity – RN Case Manager

Join a collaborative care coordination team where your expertise in case management will play a vital role in supporting safe, efficient, and patient-centered care across the hospital.

Position Details:
Start Date: 3/31/2026
Location: Sacramento, CA
Schedule: 5x8 Days
Contract Length: 13 Weeks
Floating: May float to various inpatient units (excluding surgery)

Key Responsibilities:
Assess patient needs and coordinate comprehensive care plans across departments. Facilitate discharge planning and ensure safe transitions of care. Collaborate with physicians, social workers, and multidisciplinary teams to remove barriers to care. Monitor patient progress, coordinate resources, and educate patients and families on care plans and follow-up requirements. Manage a caseload of approximately 15–25 patients daily depending on census.

Unit Details:\nPatient population includes cardiac, surgical, oncology, and critical care. EMR: Cerner. Orientation includes one-on-one training with an RN Care Coordinator.\n\nRequirements:\nActive RN license, BLS (AHA), and recent Care Coordinator experience within the health system required.
