Position Summary : 4 Your Peace of Mind, LLC is seeking a compassionate and reliable Licensed Practical Nurse (LPN) for a short-term private duty assignment in Bolton, CT. The LPN will provide direct, patient-centered care under RN supervision in accordance with the Connecticut Nurse Practice Act and agency policies.

Qualifications :

Active, unrestricted Connecticut LPN license

Graduate of an accredited practical nursing program

1+ year of home health or related experience preferred

Reliable vehicle, valid driver’s license, and auto insurance

Strong communication, documentation, and time management skills

Responsibilities :

Provide hands-on patient care and education

Administer medications and treatments per orders

Document all visits and MARs accurately within 24 hours

Report any changes in patient condition to supervising RN

Adhere to infection control and safety protocols

Use EVV system for visit verification

Location : Bolton, CT Schedule : Monday–Friday, 8 : 30 AM–4 : 30 PM Duration : 11 / 17 / 2025 – 1 / 4 / 2026
