Responsibilities

RN Case Managers are primarily responsible for providing professional nursing care to assigned patients with input from the IDT (interdisciplinary team). The RNCM evaluates, develops, implements the plan of care for each of their assigned patients.

Primary Service Locations : All of San Diego and south Riverside county.

Work Locations : Escondido, Mission Valley, Carlsbad, Temecula

Hours :   Varies

On-Call : Rotation

Weekends : Rotation

Status : Full Time - benefits eligible

Salary : $40.41- $56.66 depending on experience
