RN Case Manager - Home Care in Minnesota

Position Overview
As an RN Case Manager in Minnesota, enjoy true autonomy while making a profound difference. Visit patients one-on-one in their familiar home environments, conducting comprehensive head-to-toe assessments, coordinating care, managing diseases and medications, performing skilled wound care, catheter / ostomy management, infection prevention, and home safety evaluations. Work collaboratively with interdisciplinary teams to optimize outcomes in the southeast Twin Cities Metro territory.
Competitive Compensation & Benefits
- Annual Salary Range : $89,419 - $106,620
- Mileage reimbursement or company car program
- Generous PTO, holiday pay, and 401(k)
- Comprehensive medical, dental, vision insurance with HSA company contribution
- 100% clinical license reimbursement
- CEU reimbursement up to $400 / year + 2 paid CEU days
- Scrubs allowance
Schedule : Full-time days, Monday-Friday with holiday rotation (about 2 / year); weekend rotation may apply based on staffing.
