Co mmunity Healthcare of Texas is currently recruiting for a Social Worker.

POSITION SUMMARY

To provide therapeutic / emotional support, education, and resource information to patients and their families; Advocate for patient’s needs / wishes with team members, family members, caregivers, and other providers.

ESSENTIAL FUNCTIONS

• Assess the psychosocial needs of the patient and family members
• Provide therapeutic counseling and emotional support to patients and family members and education on end of life issues
• Contribute to and execute the Plan of Care with family / patient
• Record daily documentation of visits and contact in accordance with Policy
• Provides referrals to community resources
• Performs in-office / non-clinical administrative duties
• Perform other duties as assigned
