Job Description

The Registered Nurse Case Manager (RNCM) will establish a well-rounded care practice that includes patient assessment, care planning, implementation, coordination, and monitoring, as well as ongoing routine evaluation to ensure medically and financially appropriate care. This position requires close collaboration with the interdisciplinary team to facilitate care coordination and intensive medical management of participants recently discharged from acute care centers and skilled nursing facilities.

Responsibilities

• Facilitate interdisciplinary team care coordination.
• Perform intensive medical management of patients discharged from acute care settings.
• Collaborate with Social Worker and Physician to collect necessary medical and social information.
• Conduct assessments at participant residences as needed; mileage will be reimbursed.
• Utilize charts and documentation systems, specifically EPIC.
