Overview

Exciting Full-time Advanced Practice Provider opportunity available! Manage chronic conditions within your patient panel, seeing 6-8 patients daily, Monday to Friday, with no weekends or evenings!

    • Responsibilities

    We are seeking Nurse Practitioners / Physician Assistants with experience in home health, geriatrics, hospice, or primary care. Our ideal candidate is a passionate healthcare provider who prioritizes patient care over volume, dedicating ample time to understand and address patients’ needs.

    Qualifications

  • Licensed as a NP / PA in the relevant state
  • Federal DEA License / eligibility
  • Preferred : Geriatric training / experience
  • Active CPR / BLS Certification
  • Board Certification
