License : RN - Registered Nurse (Single State) or RN - Multi-state Compact Certifications : BLS - Basic Life Support Must-Have : Strong critical care nursing skills. Computer / data entry experience. Ability to communicate with various members of the healthcare team. Basic operational knowledge of Excel and Word. Epic and 3M experience preferred. Description : The RN Home Health is responsible for providing an initial, resumption, and recertification evaluation of the Home Health patient in their home setting. This role includes age-appropriate interventions that meet the physical, emotional, developmental, psychosocial, and educational needs of patients. The RN Home Health is responsible for the coordination of initial disciplines to begin care. They will plan care throughout the patient episode, collaborating with the physician, patient, family members, and team manager to develop the expected patient visits. Scheduling visits to the appropriate team member and incorporating therapeutic, preventative, and rehabilitative nursing actions is crucial. The RN Home Health communicates with all team members including RNs, LPNs, therapists, social workers, home health aides, the physician, the patient, family, and other individuals to ensure that the patient's plan of care is appropriately developed. Core Responsibilities : Assesses, coordinates with other disciplines, and manages the patient's total needs. Develops patient care plans in coordination with other disciplines. Performs routine and complex nursing procedures to meet patient needs. Collaborates with the physician, patient, family, and team manager to develop and review the plan of care. Maintains communication with the physician regarding any changes affecting the patient's recovery. Provides patient and caregiver education to encourage self-care and promote positive outcomes. Communicates patient status changes and lab results to the physician as appropriate. Participates in care team conferences and is responsible for checking communications regularly. Demonstrates knowledge and applies regulatory guidelines appropriately. Completes all documentation accurately and within established timeframes. Participates in the orientation and education of new employees as appropriate. Attends continuing education and stays current in clinical knowledge for the care of the home health patient. Participates in performance improvement activities to enhance department performance and care. Meets established productivity standards and participates in weekend and call rotations as required. Performs other duties as assigned by leadership as needed.
