Agawam East Rehab & Nursing is seeking a Social Services Director to fill a full-time position at our skilled nursing facility.
The Social Services Director will manage the social work, admissions, and discharge planning functions. This includes :
- Reviewing admissions documents for appropriateness of placement in our facility
- Completing social services assessments, including but not limited to BIMS, PHQ-9, trauma screening
- Planning aftercare for residents discharging from the facility, including but not limited to housing, home care services, medical equipment, and follow up medical appointments
- Maintaining open lines of communication with residents and families regarding concerns
- Coordinating care plan meetings
- Maintaining advanced directives
- Completing documentation of relevant tasks in PointClickCare
