[job_card.job_description]PM / Evening Shift (exact hours can be customized; e.g., 2 : 00 p.m. - 10 : 30 p.m.) Rotating weekends and holidays as scheduled Full-time position with consistent hours Provide comprehensive nursing care to residents, including assessments, medication administration, treatments, and monitoring of changes in condition Perform focused and head-to-toe assessments and communicate timely with providers and families when status changes Coordinate resident care with caregivers, therapists, and other team members to ensure a smooth transition from day shift to night shift Offer calm, reassuring support to residents and families during evening routines, including pain management, comfort care, and emotional support Complete accurate and timely documentation, follow care plans, and support care conferences as needed Provide clinical leadership on the PM shift by supporting CNAs / care staff, answering clinical questions, and modeling safe, person-centered care Champion infection prevention, safety, and quality standards throughout the building Current Registered Nurse (RN) license in the State of Wisconsin Previous experience in assisted living, long-term care, memory care, or similar setting preferred, but strong new RNs with solid clinical skills are welcome Strong assessment, critical thinking, and prioritization skills-especially during busy PM hours Compassionate communicator who can build trust with residents, families, and coworkers Reliable, professional, and able to work independently while being a strong team player Competitive pay for PM shift (shift differentials may be available) Benefits package for eligible staff (which may include medical, dental, vision, PTO, and retirement plan, depending on employer offerings) Supportive onboarding and training to help you feel confident in your role A mission-driven culture where your voice and ideas are valued
