Position Overview

We are seeking dedicated and compassionate Licensed Practical Nurses (LPNs) to join our home care team. As an LPN, you will provide one-on-one nursing care to clients in their homes, assisting with medical needs, monitoring health conditions, and ensuring the highest standard of care.

Responsibilities :

✅ Provide direct patient care in a home setting

✅ Administer medications and treatments as prescribed

✅ Monitor and document patient health status and report changes to RNs or physicians

✅ Assist with activities of daily living (ADLs) such as bathing, dressing, and grooming

✅ Educate clients and their families on health conditions and proper care techniques

✅ Ensure compliance with care plans and follow all healthcare protocols

✅ Collaborate with caregivers, families, and healthcare professionals to optimize patient outcomes

Requirements :

🔹 Active Florida LPN License

🔹 CPR Certification (American Heart Association or Red Cross preferred)

🔹 Minimum of 1 year of nursing experience (home care experience preferred)

🔹 Strong clinical and assessment skills

🔹 Compassionate, patient-focused, and reliable

🔹 Ability to travel to client homes within assigned locations (Port St. Lucie, Cutler Bay, Fort Lauderdale)
