As a Registered Nurse, you will provide direct nursing care according to established policies, procedures, and protocols. Your responsibilities will include :
Implementing and monitoring patient care plans, while documenting and communicating patient condition appropriately.Coordinating care among multiple disciplines for a well-rounded patient experience.Executing physician and nursing orders effectively.Assessing and coordinating discharge planning needs with the healthcare team.Demonstrating the ability to assess patients holistically, considering their physical, psychosocial, cultural, spiritual, and age-related needs.Delegating care tasks to other nursing personnel based on their competence and expertise.Participating in daily interdisciplinary patient rounds, providing valuable insights to achieve positive outcomes.Adhering to the ANA Scope and Standards of Nursing Practice, Code of Ethics, and Social Policy Statement.Developing, implementing, and revising individualized care plans.Engaging in ongoing professional growth and development beyond hands-on patient care.Additional duties as assigned.Qualifications :
Experience in educating patients on lifestyle changes to enhance cardiovascular health and prevent future heart issues.Proficiency with various exercise equipment, analyzing patient data, and working collaboratively within a multidisciplinary healthcare team.Graduate of an accredited nursing school with an associate degree required; BSN preferred.Current Connecticut RN license.BLS certification by the American Heart Association.Strong assessment and critical thinking skills.Experience in providing care for patients recovering from heart-related conditions, such as heart attacks or surgeries.As a Magnet-designated facility, St. Vincent's Medical Center is committed to nursing excellence.
