Family Nurse Practitioner
Schedule / Hours : M-F 730am-430pm
Location : Integrated Clinic 337 E. Virginia Ave Bldg B, Phx 85004
POSITION SUMMARY :
The Family Nurse Practitioner is a key member of the Integrated Health team, engaged in providing medical services and helping to define the medical direction of the organization. The position provides primary care medical services to adults to ensure the delivery of affordable quality healthcare services. The Family Nurse Practitioner works collaboratively with the Integrated Health staff to ensure clients transition seamlessly between medical and behavioral health services.
Clinical Duties
Provides full-scope outpatient primary care services, including assessment, diagnosis, treatment, and follow-up for acute and chronic conditionsProvides primary care services in the medical clinic, at NAC residential facilities, in the community at NAC housing sites, as well as telehealth visitsPrescribes medications, administers immunizations, and educates patients on health and wellnessOrders, performs CLIA waived laboratory testing, and interprets diagnostic tests and lab workProvides patient and family education focused on health promotion and disease preventionDocuments all patients encounters accurately and timely in the electronic health record (EHR)Collaborates with Behavioral Health professionals for care coordination of shared patients as neededEnsures clients have knowledge and access to medical services after hours for emergencies and urgent medical needsSupports nursing staff at residential sites with clinical oversightTakes all steps necessary to ensure accuracy of patient data, test documentation, and recordkeeping, including updating the electronic medical record with testing orders and reporting results to state, federal and other reporting agenciesEnsures a safe working environment by adhering to the standards set by Occupational Safety and Health Administration and Native American Connections policiesComplies with all patient and data confidentiality policies and regulations, including, but not limited to HIPPAKeeps abreast of emerging models in health care delivery; identify and define new and innovative strategies to achieve business goals and objectivesIdentifies opportunities to collaborate and develop clinical integration opportunities with health delivery systems to achieve affordable outcomesCompletes disability certifications as neededConducts telehealth appointments as necessaryAdministrative Duties :
Reviews and sign lab / imaging results.Reviews and signs incoming medical records.On-call availability to residential sites for after-hours questionsCreates, reviews, and signs medically related policies and proceduresParticipate in the following :Quality Management Committee, including quality improvement and compliance

Master's degree in NursingAZ Board of Nursing qualification as a Family Nurse Practitioner with an active and unrestricted licenseTwo years' experience providing medical services Experience in public sector healthcare or non-profit community health, with knowledge of Medicaid populations; experience working with Native Americans preferred.Current Fingerprint Clearance Card and DEA licenseBLS Certification.AZ Medicaid provider or ability to complete and submit application within first 30 days of employmentSKILLS REQUIREMENT :
Must possess knowledge of clinical care and client treatmentMust have prescribing and dispensing authority granted by the state board of nursing.Must have an ability to effectively use an Electronic Medical Record.Must be a team player, flexible, highly ethical, value-driven, results-oriented and a professional of high personal integrityDemonstrated skill in the use of computers, Microsoft Office, email systems, as well as in overall written communicationNATIVE AMERICAN PREFERENCE : Preference is given to qualified Native American Applicants in accordance with the Indian Preference Act. If claiming a preference, a copy of valid documentation will be necessary.

FAIR LABOR STANDARDS ACT : This position is considered to be Non-Exempt for overtime pay provisions as provided by the Federal Fair Labor Standards Act (FLSA) and any applicable state laws. Non-Exempt employees are entitled to overtime pay for hours worked in excess of forty (40) hours per workweek.
