Helio Health is on a mission to treat and promote recovery from the effects of substance use, mental health disorders, and other behavioral healthcare issues. To support the mission of Helio Health, the Service Coordinator will provide services to residents with mental health and substance use disorders. The Service Coordinator will work on a Full-Time basis out of Helio Health's Homestead CR-SRO.

Work schedule:

- Monday 2:30pm - 11:00pm.

- Tuesday and Wednesday off.

- Thursday - Sunday 2:30pm - 11:00pm

Pay rate: $18.00- $20.75 per hour.

Documents files in an accurate, concise, and professional manner to meet NYS and Federal licensing requirements.
• Actively seeks involvement of residents and significant others in program planning and progress assessments.
• Provides services which are intended to assist residents to fulfill their goals and expectations.
• Assists and teaches Activities of Daily Living (ADL) skills to residents.
• Completes service plans for residents in conjunction with the Supervisor, Senior Specialists, residents, and collateral contacts as directed by the resident.
• Reviews medication book on a daily basis to ensure that all medications have been received as prescribed.
• Ensures all medications are ordered and delivered to the Residence when necessary.
• Plans and implements on-going recovery recreation/socialization activities with residents both within the residence and in the community.
• Maintains confidentiality of all case records and patient information in accordance with HIPAA Regulations.
• Provides transportation to residents as directed.

• High School Diploma or GED required, Associate's Degree in Human Services preferred.
• HUD experience for income base housing preferred.
• At least one year experience in a mental health setting and/or experience working with individuals abusing and/or addicted to alcohol and/or drugs, is preferred.
• Must possess a valid NYS Driver's License.
