This care is based on nursing theory and research for a designated group of patients and significant others. Delivers patient care by incorporating the principles of Relationship Centered Care. Applies the nursing process based on theory, research, evidence-based practice, and approved organizational nursing standards. Assesses and evaluates patient care needs while applying critical thinking skills in patient care management. Integrates relevant assessment and intervention skills in the delivery of nursing care. Collaborates with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care. Prioritizes all aspects of patient care, including teaching, rounding, coaching, and planning after hospital care. Delegates to others as appropriate. Educates patients and caregivers about their plan of care, health promotion, and disease prevention. Communicates effectively and professionally with patients, families, and all members of the interdisciplinary patient care team. Manages assignments within the care delivery model effectively. Role models the principles of Relationship Centered Care through various patient interactions. Considers patients' values, preferences, and expressed needs in all aspects of care. Establishes and maintains therapeutic relationships with patients and families. Uses evidence-based practices to understand patients' perceptions of care. Initiates service recovery for patients and families with concerns. Demonstrates caring, respect, empathy, and active listening through dialogue and actions. Practices safe and efficient patient-centered care, identifying and escalating barriers to improvement. Demonstrates clinical skills and knowledge crucial to quality and safety in the patient population served. Implements process improvement strategies based on institutional evidence-based practices. Engages in peer and institutional review processes. Collaborates with interdisciplinary teams to create and implement quality improvement projects. Contributes to quality and safety practices and compliance measures. Implements principles of high reliability to provide a safe patient environment. Escalates potential safety hazards or gaps from best practice. Documents nursing practice accurately and thoroughly. Facilitates patient throughput on a shift basis. Contributes to an environment that supports clinical colleagues, patients, and their families. Advances clinical competence to progress from novice to expert. Acquires knowledge and skills relevant to the role and patient population. Participates in shared decision-making through specialty practice councils and unit initiatives. Experience with EPIC is highly preferred. Average unit census is 8 beds with a staffing guideline of 1 : 2. Schedule includes 3 shifts of 12 hours, every other weekend, with variable shifts. Education : Bachelor of Science in Nursing preferred. An associate degree in nursing may be considered with relevant experience or active matriculation in a BSN program.
