[job_card.job_description]Flexible Hours Great culture and team atmosphere Mileage reimbursement Employee Referral Program Employee Discount Programs 401k + Match Provide direct patient care as defined in State Nurse Practice Act Implement current nursing practice following a comprehensive assessment and the plan of care Provide accurate and timely documentation of patient services to reflect the plan of care Assess and provide patient and family / caregiver education and information pertinent to diagnosis and self-plan of care Participate in the coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group. Provide appropriate pain / symptom management. Evaluates the patient's response to treatments / medications
JourneyCare Hospice is seeking a Hospice LPN to join our growing Crisis Care team on a PRN basis! New competitive salaries and flexible hours!

HOURS : As Needed. Shifts will be 12-hour shifts 7PM-7AM both weekdays and weekends.
TERRITORY : Schaumburg, Rolling Meadows, Palatine, IL and surrounding areas.
Qualifications :
- Graduate of an accredited practical nurse or a vocational nursing program
- Three years of nursing experience. Community health, hospice or medical / surgical experience is preferred
- Currently licensed as an LPN / LVN in the practicing state
- Must possess current CPR Certification
- Demonstrates good verbal and written communication, and organization skills
- Valid driver's license with an automobile that is insured in accordance with state requirements
- Understands the philosophy of hospice concept and the needs of the terminally ill
