Key Responsibilities:

• Conduct thorough evaluations of patients' physical abilities and rehabilitation needs.
• Develop and implement individualized treatment plans tailored to each patient.
• Provide hands-on therapy to help patients achieve their rehabilitation goals.
• Monitor and document patient progress, adjusting care plans as necessary.
• Collaborate with a multidisciplinary team to optimize patient care.
• Educate patients and their families on treatment protocols and preventive care.

Qualifications:

• Doctorate in Physical Therapy (DPT) from an accredited program.
• Current Illinois state license to practice as a Physical Therapist.
• Strong communication and interpersonal skills.
• Experience in inpatient or outpatient settings is preferred, but new graduates are welcome to apply.
