This role ensures seamless coordination of clinical and non-clinical services to promote wellness, safety, and a superior resident experience. This position plays a vital role in facilitating home health services, supporting resident engagement, and providing personalized care navigation for new and prospective residents.
Job Qualifications
Education : High School Graduate
Experience : Six months to one-year-related experience required.
Skills : Good interpersonal and organizational skills. Knowledge of medical terminology, preferred. Transportation : Reliable transportation and valid and current auto liability insurance
Environmentaland WorkingConditions : Works in a health care facility in a controlled atmosphere building. Possible exposure to blood and bodily fluids and infectious diseases; ability to work a flexible schedule; ability to travel locally; some exposure to unpleasant weather.
Essential Functions
Evaluation
1. Assist with scheduling and requesting skilled nursing and therapy visits for residents.
2. Attend and contribute to daily stand-up meetings by delivering and receiving reports on home health patients.
3. Report any changes in residents’ medical status to appropriate home health clinicians in real time.
4. Lead and manage non-clinical wellness services including; staffing and oversight of instructor-led exercise classes, supervising residents during scheduled gym hours, organizing and hosting workshops such as assistive device clinics, balance screens, and wellness education courses, managing the Dementia Care Program at the facility, providing immediate customer service support to enhance the senior living experience.
5. Participate in prospective resident facility tours, providing clinical insights and support.
6. Educate families on appropriate levels of care based on clinical needs.
7. Guide residents and families through the home health onboarding process prior to or at move-in.
8. Conduct new move-in safety assessments to ensure a smooth transition.
9. Maintain equipment or furniture to keep it in good working condition, including performing the assembly or disassembly of equipment or accessories.
10. Promotes the company culture, values and mission by presenting a positive image to those we serve, including patients, families, physicians, vendors in word, action and appearance.
