Job Summary:
The Registered Nurse provides skilled nursing care, case management, and hospice care to individuals facing advancing age, frailty, and serious illness within a home health setting. This role involves patient assessments, medication administration, treatment planning, and collaboration with an interdisciplinary team. The RN is expected to manage a caseload of patients, ensuring quality care and adherence to established protocols.

Responsibilities:
- Provide skilled nursing care to patients in their homes.
- Perform patient assessments and develop individualized care plans.
- Administer medications and treatments as prescribed.
- Collaborate with the interdisciplinary team to coordinate patient care.
- Document patient care accurately and timely using electronic charting systems.
- Educate patients and families on disease management and self-care techniques.
- Manage a caseload of patients, ensuring productivity standards are met.
- Collect specimens and record vital signs, including pulse oximetry and pain assessment.
Required Skills & Certifications:
- Active Registered Nurse license.
- BCLS certification.
- Minimum of 1 year of skilled home health experience.
- Oasis experience.
- Driver's license, car insurance, and reliable transportation.
- COVID vaccination and booster.

Special Considerations:
- Traveler must have their own transportation.
- The facility can call off up to one shift per pay period.
- Local candidates within a 50-75 mile radius are accepted.
- Traveler is expected to visit up to 6 patients per day, aiming for 30 productivity points.
- Occasional travel radius of up to 60 miles per day may be required.
Scheduling:
- 8-hour day shifts.
- The facility cannot accommodate 4x10 or 3x12 schedules.
- No weekend requirements.
- On-call is not required.
- Block scheduling is not accommodated.
