Los Palos and Seacrest Care Centers are sister facilities located on 6 beautifully landscaped acres surrounded by rose gardens and trees, where residents can enjoy the breeze from the nearby Pacific Ocean.
We are looking to add per diem Occupational Therapists to our newly formed in-house Team to further develop our home-like environment and help our residents achieve their rehabilitation goals as quickly and efficiently as possible.

Responsibilities :
Evaluate residents' physical, cognitive, and psychosocial abilities, as well as functional limitations, using standardized assessments.
Develop and implement personalized treatment plans based on assessment results, resident goals, and physician orders.
Provide skilled occupational therapy interventions, which may include therapeutic exercises, training in activities of daily living, cognitive retraining, and environmental modifications.
Monitor and document resident progress, adjusting treatment plans as needed.
Collaborate with interdisciplinary team members, including physicians, nurses, physical therapists, and speech therapists, to coordinate care and ensure optimal outcomes for residents.
Educate residents and their families on occupational therapy goals, treatment plans, and home exercise programs.
Maintain accurate and timely documentation of the occupational therapy services provided.
Participate in team meetings, in-service education, and quality improvement initiatives.
Stay informed about current occupational therapy practices and relevant regulations.
Qualifications :
Current license to practice as an Occupational Therapist in the state of California.
A minimum of two years of experience in a healthcare setting, ideally in a skilled nursing facility.
Knowledge of contemporary occupational therapy practice standards and regulations.
Proficiency in using electronic medical records and other healthcare software.
