Overview

Join our dedicated team as a full-time Advanced Practice Provider! Offer personalized care managing chronic conditions in a supportive environment with a manageable patient load of 6-8 patients per day, Monday through Friday, with no weekends or evenings!

    • Responsibilities

    We are looking for Nurse Practitioners and Physician Assistants with relevant experience in home health, geriatrics, hospice, and primary care. Our ideal candidate is dedicated to practicing medicine with a strong focus on patient care rather than sheer volume, ensuring that each patient's needs are thoroughly addressed.

    Qualifications

  • Licensed as a NP / PA in the state where HarmonyCares Medical Group operates
  • Eligibility for Federal DEA License
  • Preferred geriatrics training / experience
  • Current CPR / BLS certification
  • Board certification
