Position Details :
Patient Population : Geriatric patients in nursing facilities and advanced care coordination programs
Setting : Nursing facilities and outpatient geriatric care
Responsibilities :
- Provide geriatric care across eight local nursing facilities
- Support the Trinity Health at Home Thrive Program
- Teaching opportunities with Residents, Medical Students, and Fellows
- Shared call among providers
Required Procedures : Geriatric assessments and management of complex chronic conditions
EMR System : Not specified
Support Staff : Multidisciplinary geriatric care team
Days Per Month : Full-time (Monday Friday)
On-Call / Weekends : Call shared among providers; flexibility in schedule
