Michigan Licensed Practical Nurse

A company is looking for a Remote Licensed Practical Nurse (LPN) with Michigan Licensure.

Key Responsibilities

Manage patient census with a resolution-driven approach to close gaps in patient care

Identify and coordinate community resources for patients to enhance their care

Provide patient education on managing chronic conditions and perform medication management

Required Qualifications, Training, and Education

Active Michigan License (LPN)

At least 3 years of direct patient-facing work experience

Technical aptitude with Microsoft Office Suite

Experience in chronic care management

Ability to work in a sedentary position for extended periods
