Job Summary

Licensed Practical Nurse (LPN) – SNF

Shift : Variable – Days; Variable – Evenings / Days

Hours : 8-hour shifts / 40 hours per week guaranteed

Setting : Skilled Nursing Facility (SNF)

Assignment Length : 13 weeks

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) for a 13-week assignment in a Skilled Nursing Facility (SNF) . This position offers a guaranteed 40 hours per week, with variable shifts that may include day and evening hours.

Key Responsibilities :

Deliver direct patient care in accordance with the plan of care under the supervision of an RN or physician

Administer medications and treatments as prescribed

Monitor and document patient conditions and report changes to the healthcare team

Provide support with activities of daily living (ADLs)

Collaborate with interdisciplinary team members to ensure quality patient care

Requirements :

Active LPN license in state of assignment

Minimum of 1 year of recent LPN experience, preferably in a SNF or long-term care setting

BLS certification (AHA preferred)

Strong communication and organizational skills

Ability to work flexible shifts as scheduled
