About the Role

The Carceral Transitions Case Manager (CT CM) responsible for pre-release planning and short-term case management of members releasing from city and county jails, Department of Corrections, Juvenile Rehabilitation, and Juvenile Detention facilities, ensuring they are safely and effectively transition back into community settings. Provides short-term case management services for members with highly complex behavioral health, medical, and social conditions. Working within a multi-functional team, the CT CM will connect with members, correctional facility staff, providers, caregivers, contracted vendors, community resources, and health plan partners to assess the member's reentry into the community, identify care needs, and ensure access to appropriate services to achieve positive health outcomes.

Compensation and Benefits :

The position is FLSA Non-Exempt and is eligible for overtime and has a 10% annual incentive target based on company, department, and individual performance goals. The base pay actually offered will take into account internal equity and also may vary depending on the candidate’s job-related knowledge, skills, and experience among other factors.
