POSITION SUMMARY - RN Case Management

POSITION DUTIES - Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans / Resource Utilization

MINIMUM REQUIRED QUALIFICATIONS - TX / compact RN license

BLS

BSN

2+ years recent acute care hospital case management experience (either UR or care coordination)

solid Med / Surg and Orthopedics or Unit backgrounds

PREFERRED QUALIFICATIONS - CCM or ACM

MIDAS+ and / or InterQual experience

LENGTH OF ASSIGNMENT - 13 weeks

SHIFT / HOURS PER WEEK - M-F 8-5 (40hrs / wk)

SYSTEMS - Meditech / MIDAS+

START DATE - 1 / 19 / 2026
