New Franklin Center for Rehabilitation and Nursing -
We are seeking a compassionate, organized, and regulatory‑focused Social Worker to join our interdisciplinary care team in a New York State–licensed nursing home. The Social Worker is responsible for meeting residents’ psychosocial needs, supporting adjustment to long‑term care, coordinating discharge planning, and ensuring compliance with New York State Department of Health (NYSDOH) and Centers for Medicare & Medicaid Services (CMS) regulations.
Essential Duties and Responsibilities
Complete initial, quarterly, annual, and significant‑change psychosocial assessments in accordance with NYSDOH and CMS requirements.
Provide counseling and psychosocial interventions to residents and families.
Assist residents with adjustment to institutional living, illness, disability, and end‑of‑life issues.
Participate in interdisciplinary care planning, resident care conferences, and QAPI activities.
Coordinate discharge planning and safe transition to the community or alternate level of care in compliance with NY regulations.
Serve as liaison between residents, families, facility staff, hospitals, and community agencies.
Investigate, document, and assist in resolution of resident and family concerns and grievances.
Assist with advance directives, health care proxies, MOLST forms, guardianship issues, and referrals to community resources.
Maintain timely, accurate documentation in the electronic medical record.
Participate in state surveys, audits, and regulatory reviews as needed.
Maintain confidentiality in accordance with HIPAA and New York State privacy requirements.
New York State Qualifications
Bachelor’s Degree (BSW) or Master’s Degree (MSW) from an accredited school of social work, or other degree related to healthcare, education or social services
Prior experience in a nursing home, long‑term care, or healthcare setting preferred.
Working knowledge of NYSDOH regulations, CMS requirements, and residents’ rights.
Strong communication, assessment, and documentation skills.
Ability to work effectively with elderly residents, families, interdisciplinary staff, and regulatory agencies.
