PURPOSE : The primary purpose of your job position is to assist in the planning, implementation, and evaluation of recreational, social, intellectual, emotional and spiritual programs, in accordance with the resident's assessment and care plan, and as may be directed by your supervisor.
DELEGATION OF AUTHORITY : As Activity Assistant, you are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties.

GENERAL DUTIES AND RESPONSIBILITIES :
1) Assist in planning, developing, organizing, implementing, and evaluating the activity programs of this facility.
2) Observer resident attendance, mood, behavior, and degree of involvement so that facility activities and resident progress are evaluated, by noting, reporting, and charting resident behavior.
3) Participate in community planning related to the interests of the facility and the services and needs of the resident and family.
4) Safely assist Resident who prefer to smoke in designated smoking area of the facility.
5) Ensure residents follow the smoking policy.
6) Provide smoking gown when indicated
7) Review and ensure residents who smoke are following the smoking policy and schedule. Interview residents or family members to obtain activity information.
9) Involve the resident / family in planning activity programs when possible.
10) Assist in arranging transportation to other facilities when necessary.
11) Refer resident / family to appropriate social service personnel when the facility does not provide the services or needs of the resident.
12) Assume the authority, responsibility, and accountability of Activity Assistant.
13) Assist in developing, implementing, and maintaining an ongoing quality assurance program for the activity department.
14) Ensure that all charted activity progress notes are informative and descriptive of the services provided and indicated the resident's response to the service.
15) Befriend residents to encourage resident self-esteem by visiting and conversing with residents on a regular basis.
16) Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the facility.
17) Assist in scheduling movies, planning parties, and providing games / activities for resident.
18) Encourage the resident / family to participate in the development and review of his / her plan of care.
SPECIFIC REQUIREMENTS :
1) Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.
2) Must possess the ability to make independent decisions when circumstances warrant such action.
3) Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies / personnel and the general public.
4) Must possess leadership ability and willingness to work harmoniously with other personnel.
5) Must have patience, tact, cheerful disposition and enthusiasm, as well as be willing to handle residents based on whatever maturity level at which they are currently functioning.
6) Must possess the ability to seek out new methods and principles and be willing to incorporate them into existing activity services.
7) Must be able to relate information concerning a resident's condition.
8) Must not pose a direct threat to the health or safety of other individuals in the workplace.
9) Must be able push, pull and lift at least 50LBS.
10) Must be able bend, stoop, and reposition residents in W / C.
