Job Summary :
The Licensed Practical Nurse (LPN) is responsible for direct patient care per physician instructions and in keeping with license training limitations. The LPN routes and maintains patient flow, delivers care and handles emergencies as they occur

Assesses patients' condition and medical needsAssists in performing waived laboratory testsAttends nursing in service and continuing education programsEvaluates walk-in patients and makes appropriate decisionsIdentifies primary patient problem(s) and classifies problems as emergent, urgent or non- urgentInforms providers of abnormal, acute or threatening conditionsMaintains a clean, pleasant and safe clinic environmentMaintains accurate and legible patient information and recordsManages the care of patients in collaboration with other health professionals and members of the healthcare team in order to make decisions about treatment and attain health goals.Oversees tracking of referrals and tests performed off siteProvides medical care, education, instruction and referrals under supervision of providerReceives and responds appropriately to telephone inquiries from patientsRecords and documents patient information and encounters per protocolResponds to requests for medication refills using clinical protocolsReviews lab, x-ray and emergency room reports and responds as appropriateUsing established protocols, summons patients and prepares them to be seen by providersWorks closely with team members to prepare pre-visit paperworkPerforms other duties, as requiredCompetencies

must be a graduate of an approved school of nursingCPR certifiedCurrently licensed to practice in the State of IllinoisTwo (2) years of successful experience in a clinical setting preferredPhysical Requirements :
Ability to stand for extended periods of time, hand / eye coordination and manual dexterity necessary to deliver medical services and use equipment.Ability to work in conditions that include exposure to blood, bodily fluids and tissues, contagious diseases and / or other potentially hazardous materialsFine motor coordination sufficient to perform manipulative skillsModerate physical exertion including frequent walking, standing, bending, reaching and carrying moderately heavy loads (up to 25 lbs.)
