Illinois Licensed Case Management Coordinator

A company is looking for a Field Case Management Coordinator.

Key Responsibilities

Coordinate field case management activities for clients

Assist with discharge planning and case management processes

Work independently while managing multiple cases and documentation

Required Qualifications

Bachelor's degree or non-licensed master's degree in behavioral health or human services

At least 2 years of experience in behavioral health or social services

Preferred experience in case management and discharge planning

Managed care experience is preferred

Proficiency in MS Office and ability to navigate multiple systems
