Knowledge of resources for elderly and disabled adults with mental illness, intellectual developmental disabilities and substance abuse. Ability to read and understand legal, medical and mental health documents. Ability to prioritize and respond to needs of a large caseload. Ability to identify issues, obtain relevant information, relate and compare data from different sources, and identify alternative solutions. Knowledge of and ability to use effective approaches for choosing a course of action or developing appropriate solutions and/or reaching conclusions. Ability to take action consistent with available facts, constraints, and anticipated consequences. Ability to develop and maintain effective relationships with others in order to encourage and support communication and teamwork. Ability to identify problems, determine possible solutions, and actively work to resolve the issues. Ability to be relied upon to ensure that projects within areas of responsibility are completed in a timely manner. Ability to monitor programs and/or activities and take corrective action when necessary. Ability to firmly adhere to codes of conduct and ethical principles. Ability to make decisions without regard to that personal prejudices, biases, and experiences.
Job Description: This position is responsible for conducting comprehensive psycho-social evaluations and assessments through onsite interviews with disabled adults and alleged perpetrators, telephone and personal interviews with collateral contacts. Developing and maintaining treatment plans. Providing ongoing case management upon substantiation of cases including, but not limited to, arranging for medical intervention, placement out of the home, home care, adult day care, pastoral care, meals, home repair, and financial intervention. Referring to and facilitating services of mental health and substance abuse intervention. Testifying in guardianship and other court hearings. Ongoing documentation of case progress to assess goal achievement. Ongoing crises interventions and negotiations with hostile caregivers until client is no longer being abused, neglected and/or exploited or self-neglecting within Adult Protective Services' mandated time frame guidelines.
