General Duties
- Coordinates written and verbal communication relating to the medical treatment plan between the patient, family, physician, physician staff, and other team members. Communicates and coordinates the treatment plan to internal and external departments and agencies, as needed, to establish and maintain the continuity of patient care. Assists patients in understanding their diagnosis, treatment options, and the resources available. Reviews medical documentation to assist physicians in obtaining complete and accurate documentation of the medical treatment plan. Serves as an essential link between patients and all other care providers.
- Facilitates appointments for consults and support services within established service standards, including social workers, PT, OT, ST, dietitians, physicians, and financial resources. Follows patients throughout the care continuum, including inpatient admissions, and collaborates with inpatient care management resources. Works with service line personnel to collect data, track outcomes, and support the strategic planning process. Provides nursing care and guidance to the patient from screening to survivorship.
- Performs nursing process : assessment, planning, implementation, and evaluation. Helps patients and families navigate the system, provides education, and initiates and documents patient teaching based on assessed needs. Uses appropriate documentation regarding disease process, side effects of treatment, and community resources. Supports patients during difficult decision-making periods. Works in an organized and time-conscious manner.
- May initiate and review policies related to the service provided; where appropriate, updates or writes new policies. Serves as a resource for community educational events and staff education.
Education Qualifications
Required : Bachelors degree in Nursing (BSN) or must be able to obtain a BSN within three years of hire.Experience Qualifications
Required : 2 years of professional nursing experience in a specialty area.Licenses and Certifications
Required : Louisiana State Board of Nursing - Registered Nurse LicenseRequired : American Heart Association - Basic Life Support (BLS) CertificationWork Shift
