About The Position

Description :

Provides, coordinates, and directs the provision of hospice care according to physician?s orders based on agency policies and procedures, through the competent application of the nursing process.Responsible to

Clinical Manager, Directly Supervises and Evaluates : Hospice Assistants and Licensed Practical Nurses / Licensed Vocational Nurses. State Specific-JOB DUTIES / KNOWLEDGE - Arizona State Specific : Evaluate and assess patients medical condition as ordered and will report any significant change in condition immediately to physician for medical intervention and follow-up. Records communication in patient?s medical record, including date, time, and person to which report was given. -Virginia State Specific : 1. Performs in-home supervisory visits for services provide by LPN?s and Hospice Assistants and other support staff and contract staff when such staff are present in the home and is delivering patient services. Supervisory visits of Hospice Assistants shall be performed as frequently as necessary but not less than once every two weeks. The nurse will document if Hospice Assistant services are appropriate and consistent with agency policies and procedures. Documentation shall be provided of any corrective actions taken if services are found to be unacceptable. 2. Completes updated Plan of Care every 60 days and communicates changes to attending physician, hospice staff, and contractors providing patient care. Home visits. Periodic office meetings. Must have reliable transportation, current driver?s license, and agency-required liability insurance.'

'1. Current and unencumbered license to practice as a Registered Nurse specific to that state the employee is assigned to work by the company.

2. Nurse Licensure Compact (NLC) documents the following : The mutual recognition model of nurse licensure allows a nurse to have one license (in his or her state of residency) and to practice in other states (both physical and electronic), subject to each state?s practice law and regulation. Nurses must legally reside in a NLC state to be eligible to have a multi-state license. See list of Nurse Licensure Compact (NLC)

3. Minimum of one (1) year of nursing experience required.

4. Current CPR certification.

4. Able to assess patient status and identify requirements relative to age specific needs.

5. Must be familiar with general use and functions of the computer, such as, user names and password concepts; internet; e-mail; navigation of computer desktop, including starting programs, using files, and windows, effectively use navigation buttons and tool bars; ability to self-manage online HR services and online training programs.'

State Specific-QUALIFICATIONS -Louisiana Specific : Minimum of two years? full time experience as a registered nurse. -Virginia State Specific : 1.
