Providing skilled home nursing care in accordance with physician orders and agency protocols, this role requires expertise in patient assessment, care planning, and supervision of home health aides and licensed practical / vocational nurses. Accountability to the Clinical Manager includes ensuring quality care delivery and compliance with state-specific regulations in all assigned visits.

Key responsibilities include evaluating patients medical conditions, documenting communications and changes, supervising and assigning duties to Home Health Aides and LPN / LVNs per regulatory guidelines, and maintaining up-to-date patient care plans. The nurse will perform home visits regularly and participate in periodic office meetings, necessitating reliable transportation, a current drivers license, and agency-required insurance.

Qualifications and Desired Experience :

• Current, unencumbered Registered Nurse license valid within the assigned state
• Nurse Licensure Compact (NLC) eligibility preferred for multi-state practice
• Minimum of 2-3 years nursing experience, ideally home health or community-based care
• CPR certification current and valid
• Ability to assess patient status accurately with knowledge of age-specific needs
• Computer proficiency including email, internet navigation, and basic software use
• Compliance with state-specific licensure and practice standards (e.g., Illinois, Arizona, Oregon, Wisconsin, Nevada)
