LPN-Field Admissions Polk, Hardee, Highlands county

Tidewell Hospice a member of Empath Health is seeking an LPN to join the Empath team.

This position is Monday - Friday 10am - 6pm for Polk Hardee Highlands county.

The LPN :

• Under the direction of the Access Manager and the Hospital Access Navigator the LPN is responsible for identifying Patient / Family needs and coordinating Interdisciplinary Team (IDT) provision of services under physicians orders.
• Utilizes monitoring intervention and teaching skills to maximize patient comfort and enhance the quality of life for the Patient / Family unit.
• The LPN is supervised and collaborates with Primary RN and / or CTM.

Position Requirements

Valid Florida LPN License

Valid CPR certification from the American Red Cross or American Heart Association
