Health Coach in Care management team

Hiring health coaches, phlebotomists, EMTs, medical assistants, dental assistants, hygienists, RNs, LPNs, and pre-med students to conduct at-home patient surveys. Assessments are available in Upstate New York.

Scope of work:

The care management activities are designed to promote wellness and preventative care, improve member quality of care and health outcomes, reduce hospitalization, and comply with all regulatory requirements.

Health Coaches: Perform in-person care management visits, assist with benefit coordination, order routine supplies, troubleshoot supply and DME delivery issues, and notify clinical staff of identified issues and clinical triggers requiring escalation.

Fee for service.

At AHS you are an independent contractor and will receive a 1099. We pay per patient. Compensation for the 30 min screening is $40-$70 depending on how many tests are administered.

Applicants will be expected to complete the following and provide copies of :

-Driver's license or Government issued ID

-Copy of Social Security card

-Application

-Medical history complete with -vaccination history

-Copy of the voided check

One-day training is mandatory

Training will consist of:

Infection control training

Paperwork/software training

Specific tests
