[job_card.job_description]Responsible for reviewing and making recommendations on consumer assessments for long term care services and supports Maintain current knowledge of regulatory changes pertaining to quality and validity of assessments Assess cases presented and utilizes clinical competencies to review and validate assessment outcomes Work collaboratively to monitor and address clinical quality and gaps in care Provide clinical and educational assistance to support project initiatives Demonstrate communication skills necessary to interpret age-specific responses Participate in performance, improvement, and quality assurance activities and related meetings Provide clinical and educational support to meet program goals Meet all standards established for this position as outlined in the corresponding annual performance criteria and bonus template for this position Perform other duties as may be assigned by Management Current NY Nurse Practitioner Licensure Proof of Board of Certification Proof of Malpractice Insurance showing professional liability coverage of 1M per incident / 3M agg 2+ years practicing experience as a Nurse Practitioner Computer literacy and the ability to navigate new and emerging technology Must meet device requirements Highly developed written and oral communication skills; strong interpersonal skills Ability to work in a fast-paced and changing environment with proven ability to work with community groups Physical exam signed and dated by a physician within the last year PPD or QuantiFeron (within 1 year) or chest X-ray with Questionnaires (every 5 years) TDAP within 10 years Quantitative Titers for MMR & Varicella Hepatitis B vaccination schedule or Declination Form Also Accepted Proof of Covid19 Vaccine Proof of Flu Vaccine
