We're looking for an Occupational Therapist who is passionate about helping older adults and individuals with disabilities thrive safely and independently at home. As we expand into new markets, we're looking for someone who can travel as needed to meet our growing demand. A successful candidate is highly empathetic, can juggle the needs of our clients, programs and business, and is eager to make a lasting impact. We're a tech-first team so comfort working with new systems and tools will be an advantage. Also a huge plus if home modification work is part of your past / current experience.

Complete required onboarding and credentialing needsParticipate in ongoing training & development programming as neededConduct in-home assessments and functional evaluationsDeliver personalized, evidence-based recommendations for home modificationsEducate clients and caregivers on fall prevention and safe mobility techniquesEngage clients and families in collaborative care planningMaintain accurate and timely documentation through our electronic systemSupport in ad hoc clinical initiativesQualifications
Degree in Occupational Therapy from an accredited programActive, unencumbered OT license - ideally in two of the following : MI, OH, WI, NY2+ years of clinical experienceExperience in home health, aging in place, or disability services preferredStrong interpersonal and clinical reasoning skillsReliable transportation and valid driver's licenseProficiency with technology and digital documentationAbility to work independently while collaborating with a remote teamMore About Compensation
