• Required Skills : Short Term Acute Care Inpatient Medical Social Worker, Assess response to treatment, Biopsychosocial assessment, Collect and analyze patient information, Conduct initial evaluation, Develop plan of care to meet patient / caregiver needs, Development of problem list, Establish and revise goals, Review of Health history, Collaborative care, Coordinates with internal and external team members, Documents pertinent information to facilitate care coordination, Participates in care conference activities, Refers to rehab and long term care facilities, Refers to other disciplines / resources, Documentation of care delivery, plan, goals, etc, Documentation of patient / caregiver teaching / learning, Documents patient response, Food stamps, Housing Charities, Medical Transportation, TANF (Temporary Aid for Needy Families), Child protective services, NASW Code of Ethics / Standards of Home Health Care
