A company is looking for a Care Manager Specialist to coordinate care for members with social determinants of health needs.
Key Responsibilities
Engage with members telephonically to coordinate care and support their health needs
Develop individualized care plans in collaboration with members and the interdisciplinary care team
Document care management activities accurately in the member's electronic health record
Required Qualifications
2+ years of experience in a health-related field
2+ years of customer service experience
Proficiency in Microsoft Office Suite
Access to a private space for work
Associate's degree in a health care-related field or relevant experience
