POSITION SUMMARY :
Reporting to the Team Lead or Manager of In-Patient Unit, this position is responsible for carrying out the duties assigned to increase the patient's level of comfort and to maintain personal hygiene and a safe healthy environment. Although the Home Health Aide reports to a primary manager, HEB expects the Home Health Aide may work in more than one program or department and assignments are given to best meet the needs of our patients.
ESSENTIAL FUNCTIONS :
- Provide personal care as directed by the hospice nurse.
- Completes each patient' care as delineated in the Home Health Aide care plan.
- Maintains a safe and clean environment through care of the patient's room and personal equipment and perform light housekeeping duties.
- Prepares and serves meals as instructed by the hospice nurse.
- Participates in in-services and skills training to ensure compliance state and federal regulations, as well as accreditation standards.
- Reports any changes in the patient's physical or emotional condition, including complaints of pain or change in the family's situation to the RN.
- Document daily activity and of direct care provided to the patient per HEB requirements.
- Recognize and report emergencies.
- As assigned and supervised by the hospice RN, assists the patient with activities which include but are not limited to the following :
- Attends in-services, workshops, and staff meetings.
- Assist with patient transfers and use of special equipment.
- Follow standard precautions in delivering patient care.
- Maintain comfortable environment. Is alert to safety factors and accident prevention.
- Launder patient's personnel laundry including bed linens as needed.
- Remind patient to take medications at prescribed time.
- May take blood pressure, temperature, pulse, respiration, or weight as directed by the hospice RN.
POSITION REQUIREMENTS :

Must also be able to work collaboratively within a team, have strong communication skills and be flexible with work assignments and professional when dealing with clients and staff.Must adhere to department goals, objectives, and performance standards.Able to establish and maintain cooperative relationships with internal and external customers.Must be able to understand and carry out directions and instructions and maintain patient confidentiality.Must be physically able to perform duties as assigned and have reliable transportation.EXPERIENCE AND EDUCATION :
Must complete the Home Health Aide competency test with a passing score of 75% or betterMust have completed an official Home Health Aide training course and have a Home Health Aide Certificate issued by the State Department of Public Health.If there has been a 24-month lapse in furnishing services, the individual must complete another state approved Home Health Aide program before providing services.Valid CPR certification.Benefits :
