We are a Medicare-certified home health agency dedicated to delivering compassionate and personalized care in the comfort of our patients’ homes. We are currently seeking a dependable and motivated Physical Therapist Assistant (PTA) to support patient rehabilitation and improve mobility and quality of life across San Bernardino, Riverside, and Orange County.
Responsibilities:
Provide in-home physical therapy treatments as directed by a supervising Physical Therapist
Assist with implementing care plans and documenting patient progress
Educate patients and caregivers on home exercise programs and mobility support
Communicate changes in condition or response to therapy with the PT and care team
Complete accurate and timely clinical documentation
Qualifications:
Current and valid California PTA license
Minimum 1 year of experience (home health preferred, not required)
Reliable transportation and valid driver’s license
Strong interpersonal, clinical, and organizational skills
