Position Overview :
Health Care Connectors is hiring a skilled Licensed Practical Nurse (LPN) to deliver direct care to patients of all ages in a home and outpatient setting. The LPN will support daily clinical functions, document care activities, and participate in patient education and discharge planning.
Key Responsibilities :
- Provide hands-on nursing care including medication administration and vital sign monitoring
- Assist with specialized procedures and clinical assessments
- Coordinate care plans with the healthcare team
- Document patient care using EHR systems (e.g., WellSky, HomeCare HomeBase)
- Educate patients and families on treatment plans and discharge instructions
