Delivers patient care by incorporating the principles of Relationship Centered Care. Assesses and evaluates patient care needs and applies critical thinking in patient management. Integrates relevant assessment and intervention skills in the delivery of nursing care. Collaborates with the interdisciplinary healthcare team to develop, implement, and evaluate the plan of care. Prioritizes all aspects of patient care, including teaching, rounding, coaching, and planning after hospital care. Educates patients and caregivers about their plan of care, health promotion, and disease prevention. Communicates effectively and professionally with patients, families, and all team members. Manages assignments within the Care Delivery Model and demonstrates effective allocation of resources. Supports patient experience through facilitation of a variety of actions, including admissions and bedside reporting. Considers patients' values and preferences in all aspects of care. Establishes and maintains therapeutic relationships with patients and families. Uses evidence-based practices to enhance understanding of patients' perceptions of care. Initiates service recovery for patients and families with concerns and escalates when necessary. Demonstrates empathy and active listening through dialogue, body language, and actions.
